ABSTRACT

@# In recent years, the number of lung surgeries has increased year by year, and the number of patients with postoperative cough has also increased gradually. Chronic cough after lung surgery seriously affects patients' quality of life and surgical outcome, and has become one of the clinical problems that clinicians need to solve. However, there is currently no guideline or consensus for the treatment of chronic cough after lung surgery in China, and there is no standardized treatment method. Therefore, we searched databases such as PubMed, Web of Science, CNKI, and Wanfang databases ect. from 2000 to 2023 to collected relevant literatures and research data, and produced the first expert consensus on chronic cough after lung surgery in China by Delphi method. We gave 11 recommendations from five perspectives including timing of chronic cough treatment, risk factors (surgical method, lymph node dissection method, anesthesia method), prevention methods (preoperative, intraoperative, postoperative), and treatment methods (etiological treatment, cough suppressive drug treatment, traditional Chinese medicine treatment, and postoperative physical therapy). We hope that this consensus can improve the standardization and effectiveness of chronic cough treatment after lung surgery, provide reference for clinical doctors, and ultimately improve the quality of life of patients with chronic cough after lung surgery.

ABSTRACT

ObjectiveTo evaluate the methodological and reporting quality of clinical practice guidelines for Chinese patent medicine (CPM) with internationally recognized tools the appraisal of guidelines for research and evaluation (AGEREE) Ⅱ and reporting items for practice guidelines in healthcare (RIGHT), thereby providing refe-rence for the clinical application and future development of CPM guidelines. MethodsDatabases including CNKI, VIP, Wanfang and Sinomed were searched for CPM guidelines, as well as medlive.cn, websites of China Association of Chinese Medicine and Chinese Medical Association, and reference lists of the included papers. The quality of the guidelines was evaluated using the AGREE Ⅱand RIGHT tools, and consistency tests were performed using Interclass Correlation Coefficient, and descriptive analysis and chi-square test were used to analyze the reporting rate for each domain and the average score for each item. ResultsFinally, 140 CPM guidelines were included, of which 51 were disease-oriented and 89 were drug-oriented, all of which were issued by China. For 51 disease-oriented CPM guidelines, the highest average score of all six AGREE Ⅱ domains was 73.32% for clarity, and the lowest was 26.80% for application; for 89 drug-oriented CPM guidelines, the highest average score was 55.62% for scope and purpose, and the lowest was 31.32% for rigour of development. In terms of the seven domains of the RIGHT checklist, the highest reporting rate was 68.26% for background, and lowest was 27.45% for other areas regarding the disease-oriented CPM guidelines; the highest reporting rate was 61.31% for background, and the lowest was 4.49% for other areas regarding drug-oriented CPM guidelines. The average reporting rate was higher for disease-oriented than drug-oriented CPM guidelines in three domains of AGREE Ⅱ (rigour of development, clarity of presentation, editorial independence), as well as four domains of RIGHT checklist (basic information, evidence, funding and declaration and management of interests, and other areas). ConclusionThe overall methodology and reporting quality of the current CPM guidelines still need to be improved. It is recommended that future guideline development teams should strictly refer to the AGREE Ⅱ and RIGHT checklist, and take into account of the characteristics of CPM guidelines and relevant methodo-logical suggestions in the development and reporting of CPM guidelines, thereby guiding the clinical use of CPM in a better way.

ABSTRACT

Recommendation formation is a key component of clinical practice guidelines for Chinese patent medicine (CPM), and should encompass the determination of the strength and direction of the recommendation, the rationale for the recommendation, and the methodology for implementing the recommendation. Recommendations can be formed through formal consensus and informal consensus. The strength and direction of recommendations for CPM guidelines should be determined by considering the quality of evidence for CPMs, the priority of the clinical questions, the pros and cons of efficacy and adverse effects, patient acceptance, the feasibility of the recommendation and the availability of resources, social fairness, economic benefits, and other influencing factors. In order to better guide guideline developers to consider these factors more rationally, this article provideed a detailed explanation of each factor in the context of the characteristics of TCM.

At present, there is no consensus on the diagnosis, treatment, and management of pulmonary nodules with integrated traditional Chinese and Western Medicine in China. To establish the clinical management strategy of pulmonary nodules with integrated medicine, a consensus was formed after full discussion by the experts from the Cancer Committee of Chinese Association of Integrative Medicine and the Cancer Committee of Beijing Association of Chinese Medicine according to the guidelines for pulmonary nodules issued in recent years and the clinical practice of multiple hospitals in the treatment of cancers with integrated Chinese and Western medicine. This consensus involves the existing clinical management guidelines of pulmonary nodules and the intervention methods of traditional Chinese medicine (TCM). Considering the therapeutic positioning and value of TCM, this consensus standardizes the whole-process management of pulmonary nodules with integrated traditional Chinese and Western medicine, which will help more patients.

ABSTRACT

In recent years, immunotherapy represented by immune checkpoint inhibitors programmed death 1 (PD-1) has made great progress in the treatment of esophageal cancer and is rewriting the global paradigm for the treatment of esophageal cancer. According to current data, only a small number of patients with esophageal cancer could benefit from immunotherapy. Therefore, it is a challenge to screen the potential beneficiaries of PD-1 inhibitors. Studies have shown that the expression level of programmed death-ligand 1 (PD-L1) in esophageal cancer is closely associated with the efficacy of PD-1 inhibitors, and PD-L1 is the most important predictive biomarker of the efficacy of PD-1 inhibitors. With the clinical application of different PD-1 inhibitors and PD-L1 protein expression detection platforms, clarifying the clinical significance and timing of detection of PD-L1 protein expression in esophageal cancer, and establishing a standardized PD-L1 testing procedure, are of great significance to improve the accuracy of detection and reduce the difference between laboratories, so as to maximize the therapeutic benefits for patients. This consensus was finally reached, based on the combination of literature, expert experience, and internal discussion and voting of committee members, to provide an accurate and reliable evidence for clinicians to make decisions.

ABSTRACT

The condition of patients with severe traumatic brain injury (sTBI) complicated by corona virus 2019 disease (COVID-19) is complex. sTBI can significantly increase the probability of COVID-19 developing into severe or critical stage, while COVID-19 can also increase the surgical risk of sTBI and the severity of postoperative lung lesions. There are many contradictions in the treatment process, which brings difficulties to the clinical treatment of such patients. Up to now, there are few clinical studies and therapeutic norms relevant to sTBI complicated by COVID-19. In order to standardize the clinical treatment of such patients, Critical Care Medicine Branch of China International Exchange and Promotive Association for Medical and Healthcare and Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Chinese expert consensus on clinical treatment of adult patients with severe traumatic brain injury complicated by corona virus infection 2019 ( version 2023) based on the joint prevention and control mechanism scheme of the State Council and domestic and foreign literatures on sTBI and COVID-19 in the past 3 years of the international epidemic. Fifteen recommendations focused on emergency treatment, emergency surgery and comprehensive management were put forward to provide a guidance for the diagnosis and treatment of sTBI complicated by COVID-19.

ABSTRACT

@#Cushing′s syndrome(CS)is a clinical syndrome caused by a variety of causes, with main manifestations exhibited by central obesity, purple skin striae, hypertension, and diabetes. In patients with adrenocorticotropic hormone(ACTH)-dependent CS, the result of bilateral inferior petrosal sinus sampling(BIPSS)is the gold standard for determining the source of ACTH in the absence of routine imaging findings.However, the indications, contraindications, operating procedures, precautions and outcome judgments of BIPSS differ from one medical center to another, and there are currently no international and domestic clinical guidelines and expert consensus on BIPSS. In order to further improve the operation specifications of BIPSS in the diagnosis process of difficult CS, the Hypothalamic and Pituitary Disease Group of the China Alliance for Rare Diseases and the Innovation Center of Pituitary Diseases of Peking Union Medical College Hospital organized experts in endocrinology, interventional radiology, neurosurgery and laboratory in China to formulate this consensus. This consensus aims to provide standardized methodological guidance for the use of BIPSS in the differential diagnostic process of CS in clinical practice.

ABSTRACT

RhD antigen is the most immunogenic antigens in the Rh blood group system and can cause hemolytic transfusion reactions (HTRs) and severe hemolytic disease of the fetus and newborn (HDFN). RhD-negative red blood cell (RBC) is a scarce resource in China, and RhD-negative patients face challenges in timely blood transfusion. For different populations that were initially negative for RhD (saline method), what techniques should be used, how to issue test report and when should " Asian type" DEL (RHD*01EL.01, c.1227G>A) be identified are the confusions of blood transfusion workers in the process of clinical blood supply. More than 200 technical experts discussed and reached a consensus on blood transfusion compatibility testing for Chinese population. The purpose of the consensus is to further standardize and refine the RhD blood group testing strategy, provide technical support for achieving accurate blood transfusion for patients with different RhD blood groups, continuously improve the levels of prevention and treatment for RhD-HTRs and RhD-HDFN, and lay a foundation for the future development of industry standards concerning RhD blood group testing strategy.

ABSTRACT

The killer-cell immunoglobulin-like receptor(KIR) gene family exhibits complex genetic diversity. In addition to allele level polymorphisms, KIR genes show extra diversity at haplotype content as well as copy number variation. In order to standardize the KIR gene testing, the Working Party on Histocompatibility, Chinese Society of Blood Transfusion (CSBT)and Shenzhen Blood Center organized experts to discuss and reach a consensus on KIR gene testing based on KIR-related literature and practical experience. The present consensus has summarized the techniques for identifying the diversity of KIR genes, quality control, testing report and its applications, aiming to improve the accuracy of KIR gene testing results.

ABSTRACT

Transfusion associated graft versus host disease (TA-GVHD) is a serious transfusion adverse reaction. It is widely believed that transfusion of irradiated blood is the most effective method to prevent TA-GVHD. There are some problems in the application of irradiated blood in China, such as slow development, insufficient understanding, and non-standard application. This consensus was completed after in-depth discussion by experts in the fields of transfusion medicine, hematology, and pediatrics in China. The Writing Group mainly discuss key technical points and clinical applications related to irradiated blood, and proposes 23 related recommendations. This consensus aims to standardize and promote the rational use of irradiated blood, minimize the risk of TA-GVHD in recipients and lay the foundation for developing more standardized guidelines in the future.

ABSTRACT

At present, ABO-incompatibility hematopoietic stem cell transplantation (ABOi-HSCT) accounts for 30%~50% of the total HSCT, and is no longer a major obstacle to HSCT. Transfusion therapy is an important treatment for HSCT patients during transplantation, and correct blood typing and blood selection of blood component are particularly important.For patients with ABOi-HSCT, one of the challenging issues for laboratory technician is to complete ABO blood group testing and report before, during, and after ABOi-HSCT, as well as the selection of blood components that balance transfusion safety and efficacy. This consensus was jointly discussed by domestic experts in transfusion medicine and hematology in China, and 7 recommended opinions were extracted to further standardize the blood typing and blood component infusion strategies for ABOi-HSCT patients, provide technical support for standardized blood typing reports and accurate blood component infusion for HSCT patients, and continuously improve the safety and efficacy of blood transfusion.

@#OBJECTIVE To further standardize the clinical application of anticoagulants and provide reference for improving the efficiency of prescription review of anticoagulants in medical institutions. METHODS Initiated by the Pharmaceutical Administration Committee of Chongqing Hospital Association and led by the Second Affiliated Hospital of Chongqing Medical University, in collaboration with experts in relevant fields from multiple provinces across the country, the prescription review rules, warning levels, and intervention measures for 12 kinds of common anticoagulants were established based on drug instructions. Regarding clinical drug use that was not covered by the basic review rules, based on reference to existing guidelines and expert consensus, 42 experts from 22 hospitals in 8 provinces across the country were consulted by letter to form a consensus. RESULTS & CONCLUSIONS The basic prescription review rules of 12 kinds of anticoagulants, including unfractionated heparin, enoxaparin sodium, nadroparin calcium, dalteparin sodium, fondaparinux sodium, bivalirudin, argatroban, warfarin, dabigatran etexilate, rivaroxaban, apixaban and edoxaban, had been established according to drug instruction, as well as warning levels beyond the basic review rules and corresponding intervention measures. After two rounds of expert consultation, 17 recommendations for prescription review were formed regarding some clinical drug use that were not covered by the basic prescription review rules. With the progress of clinical research, this consensus will be further updated and adjusted; in addition, as this consensus does not cover all situations where anticoagulants are used in clinical practice, prescription reviewer must conduct personalized evaluations based on the specific situation of the patient.

@#Clinically, the recurrence and metastasis of liver cancer after local ablation therapy have been the main factors affecting the long-term survival of patients with liver cancer. As a new generation of composite cryoablation and thermal ablation system, Co-Ablation System creatively integrates the advantages of profound hypothermia cryoablation and thermal ablation, which can maximize the release of tumor antigen while completely destroying liver cancer in situ, thus, the purpose of controlling liver cancer recurrence and metastasis can be achieved. Co-Ablation System treatment is usually carried out through percutaneous puncture under ultrasound or CT guidance. The selection of local anesthesia or general anesthesia is based on the size and location of the liver cancer. Co-Ablation System treatment is mainly suitable for single tumor with diameter <5 cm and multiple tumors(≤3 tumor lesions) with a maximum diameter <3 cm. After ablation, protection of liver function and symptomatic treatment are employed according to patient’s condition. About one month after ablation, MRI or contrast-enhanced CT examination is performed to evaluate the therapeutic efficacy, then, the patients are followed up regularly. In order to standardize the application of Co-Ablation System in treating liver cancer, the Committee of Minimally Invasive Therapy in Oncology and the Committee of Ablation Therapy in Oncology, Chinese Anti-Cancer Association, have organized domestic experts engaged in liver cancer ablation therapy to make a professional and in-depth discussion and to compose this expert consensus on Co-Ablation System for the treatment of primary liver cancer.

Children with certain comorbidities and immunocompromising conditions are highly vulnerable to SARS-CoV-2 infection. Vaccination against SARS-CoV-2 is an important strategy to reduce death, critical illness and overall disease burden. With the evolving and increasing transmission of SARS-CoV-2, universal vaccination is essential to achieve this goal. Children with special medical conditions are considered as the priorities for SARS-CoV-2 vaccination. However, vaccine hesitancy towards the implementation of SARS-CoV-2 vaccination currently remains an urgent challenge. In order to promote the sustainable vaccination for those children in Shanghai as well as China, Shanghai municipal center for disease control and prevention, together with the national children’s medical center, children’s hospital of Fudan university and the expert group on immunization planning of the Shanghai preventive medicine association, organized a consensus expert working group to formulate the evidence-based recommendations and implementation suggestions for children with common chronic diseases, allergy history, diseases involving adverse events related to vaccination, and immunocompromising conditions, based on the published evidence of SARS-CoV-2 vaccination for populations and children with special medical conditions.

@#With the widespread application of minimally invasive esophagectomy, inflatable video-assisted mediastinoscopic transhiatal esophagectomy (IVMTE) has gradually become one of the alternative surgical methods for transthoracic esophagectomy due to less trama, fewer perioperative complications and better short-term efficacy. However, there is no uniform standard for surgical methods and lymph node dissection in medical centers that perform IVMTE, which affects the standardization and further promotion of IVMTE. Therefore, on the basis of fully consulting domestic and foreign literature, our team proposed an expert consensus focusing on IVMTE, in order to standardize the clinical practice, guarantee the quality of treatment and promote the development of IMVTE.

ABSTRACT

@#The increasing number of pulmonary nodules being detected by computed tomography scans significantly increase the workload of the radiologists for scan interpretation. Limitations of traditional methods for differential diagnosis of pulmonary nodules have been increasingly prominent. Artificial intelligence (AI) has the potential to increase the efficiency of discrimination and invasiveness classification for pulmonary nodules and lead to effective nodule management. Chinese Experts Consensus on Artificial Intelligence Assisted Management for Pulmonary Nodule (2022 Version) has been officially released recently. This article closely follows the context, significance, core implications, and the impact of future AI-assisted management on the diagnosis and treatment of pulmonary nodules. It is hoped that through our joint efforts, we can promote the standardization of management for pulmonary nodules and strive to improve the long-term survival and postoperative life quality of patients with lung cancer.

@# In order to further regulate the application of 3D reconstruction in thoracic surgery, the Chinese Expert Consensus Group on the Application of Integrated 3D Reconstruction with Artificial Intelligence in Thoracic Surgery conducted discussions and developed this consensus. This consensus is based on the clinical experience and existing prospective or retrospective studies of 3D reconstruction technology in various scenarios of thoracic surgery and summarizes recommendations, and also appends a list of 3D reconstruction technology application scenarios that are currently controversial, not fully studied, or still in the exploratory stage, to provide direction and evidence for future clinical research and disease diagnosis and treatment, and to reach a consensus.

ABSTRACT

Maculopathy caused by various fundus diseases in the late stage is a common cause of low vision. Medical technology is difficult to reverse the loss of macular function currently, so interventions that help improve the visual system, utilize residual visual function, and improve quality of life deserve attention. Damage to the fovea of the macula does not mean that the entire retinal function is impaired. There may be one or more retinal regions adjacent to the fovea that can serve as a fixation center. It is possible to form stable paracentral fixation, complete functional remodeling of the visual system, and effectively utilize residual visual function by taking appropriate training on these potential paracentral fixation points for most patients. In 2021, a clinical guideline has been published for low vision rehabilitation in China. In order to strengthen the precise management of diseases and develop a standard operating procedure for visual training specifically for patients with low vision due to macular disease, the National Clinical Research Center for Eye Diseases initiated and organized relevant domestic experts, utilizing the latest research experience at home and abroad, and through repeated discussions, this consensus (International Practice Guideline Registration Number: PREPARE-2023CN199) was formed as a reference for ophthalmologists, optometrists and rehabilitation physicians in their clinical research and practice.

ABSTRACT

Intravitreal drug injection is a treatment for common chronic fundus diseases such as age-related macular degeneration and diabetic retinopathy. The "14th Five-Year" National Eye Health Plan (2021-2025) recommends focusing on fundus diseases and improve the management mode of patients with chronic eye diseases. Therefore, it is imperative to explore how to further optimize the service process of intravitreal injection under the premise of guaranteeing patients' medical safety, to promote medical service efficiency and standardized management level and improve the medical experience of patients. Based on the quality control standard of vitreous cavity injection for retinopathy in China, Chinese fundus disease and related field experts developed the present expert consensus on the establishment of a one-stop intravitreal injection model and the management of its organization after a serious, comprehensive, and complete discussion, focusing on a standardized operation process, quality control, and safety management, providing more references for establishing a suitable intravitreal injection management model for ophthalmology and promoting the development of diagnostic and treatment models for fundus disease in China.

Objective:To construct "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " based on the Delphi method, and to provide a methodological basis for consensus construction.Methods:After systematic search and evaluation of the literature related to chronic inducible urticaria, the first draft of "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " was written, and a questionnaire was designed for expert consultation. A representative sample of 25 experts was selected to conduct two rounds of correspondence consultation via electronic questionnaire in strict accordance with the Delphi method, and the content of the consensus was revised and improved according to the consultation results.Results:The response rates in the two rounds of questionnaire consultation were both 100%, and the expert authority coefficient was 0.92 ± 0.09. In the first round of consultation, the coefficients of variation (CV values) of 9 items were greater than 20%, and the mean agreement degree of 3 items was less than 7 points; in the second round of consultation, the CV values of all items were less than 15%, the agreement degree of the above 3 items whose mean agreement degree was less than 7 points in the first round of consultation all rose to over 7 points, and the median agreement degree of all items was greater than or equal to 8 points. Reliability analysis of the two rounds of questionnaire results showed that the Cronbach α coefficient and standardized Cronbach α coefficient were both greater than 0.9; the P values in the agreement tests by using Kendall′s coefficient of concordance for the two rounds of questionnaire results were both less than 0.001, and the Kendall′s coefficients of concordance were 0.170 and 0.219 in the first and second rounds of questionnaire consultation, respectively. Conclusion:The Delphi method-based "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " is highly representative, authoritative and reliable; this study also provides a methodological reference for the formulation and research of consensus.
